Assumption of Risks:
I, as a participant, agree by registering for the above-mentioned Trip, I knowingly and freely assume certain injury and legal risks. My participation in this trip will be deemed to indicate acceptance of such risks.
Fitness to Participate:
I acknowledge to Kanata Seniors Council Inc. that I do not have any physical limitations, medical ailments, or physical or mental disabilities that would limit or prevent me from participating in the Trip.
Waiver of Liability:
I agree to release, waive and discharge Kanata Seniors Council Inc., its volunteers or its agents from any and all liability to me, and to my heirs, executors, and administrators, that we have, or may have in the future, for any loss or damage and any claims or demands for such loss or damage on account of personal illness, injury and damage including death or property loss, however caused, including negligence on the part of Kanata Seniors Council Inc, it’s volunteers or agents, as result of my participation in this Trip. By signing below, I acknowledge that I have read and understand all information on this form. I agree to the terms above and accept the risks of participating in this Trip.
Note: Refunds will only be issued if the Kanata Seniors Council Inc. cancels the event.
